Highly trained, professional health care providers who strive to make every interaction with each resident memorable and uplifting is at the heart of Belle Plaine Specialty Care. We seek to provide the best care to enable each of our residents the greatest amount of independence possible.
Our skilled physical therapist, occupational therapist and speech therapist visit with each resident to work on their personalized care plan to improve strength, balance, mobility and overall health. Our state-of-the-art Accelerated Care Plus program is also used in our outpatient therapy sessions available to any area resident in need of therapeutic services as prescribed by a physician.
Located at the edge of Belle Plaine, our residents enjoy the benefits of small town living – quiet, peacefulness surrounded by caring neighbors who frequently join us for programs or outings. Monthly visits from members of the local historical society, musicians putting on a show, FFA members showing their animals, gardening in spring and summer, enjoying time on the patio, taking a trip down to the Dollar General or the local community center for lunch and bingo – there is always something happening at Belle Plaine Specialty Care.
We are very committed to helping our area’s senior citizens in their efforts to remain independent so we offer an Equipment Loan Closet. Making assistive devices and adaptive equipment available to the public at no cost on a temporary basis allows them to remain in their homes. Bath benches, walkers, handrails, toilet risers, etc. are available. Give us a call for more information.

Belle Plaine Specialty Care is a 66-bed skilled nursing facility offering rehabilitation services, skilled nursing care, long term care and hospice care. We are a part of Care Initiatives, Iowa’s largest not-for-profit skilled nursing and long term care provider. Belle Plaine Specialty Care accepts Medicare and Medicaid, and also serves as a contracted provider for the Veterans Affairs Community Nursing Home Program.
